To best prepare for your first visit with us, the Patient Registration Forms
must be completed and signed prior to the visit with the Healthcare Provider.

The Patient Registration Forms are sent to the email you provided.
(NOTE: our email may be sent to your SPAM folder,
so be sure to check for our email named "Fill out your patient forms online" sent from
"Healthy Wings Family and Psychia <noreply@tebra.com>".)

Patient Registration Form may be completed on a computer, laptop, tablet, or by cellphone.
BE SURE TO CLICK "SUBMIT" WHEN FINISHED.

Please thoroughly complete each form.
Be sure to select "Save As" to name your completed forms to your computer.
Please name your forms package with your LAST NAME FIRST NAME.

or print the documents and bring them with you to your appointment.

failure to complete our forms and submit all required information,
may result in cancellation of your appointment
as well as forfeiture of any fees paid.
