IMPORTANT INFORMATION ABOUT
CORONAVIRUS & YOUR APPOINTMENT
Although we are striving to remain available to serve your
healthcare needs and to provide a safe and healthy environment
for everyone during this most difficult time, please be aware of the following:
NEW TELEPHONE HOURS: MONDAY - WEDNESDAY, 9:00 A.M. TO 4:00 P.M.
1. All appointments NOT CONFIRMED WITHIN 24 HOURS OF A SCHEDULED APPOINTMENT,
WILL BE AUTOMATICALLY CANCELLED.
Please telephone the office at (520) 477-1815 if you desire to cancel or reschedule an appointment.
2. In order to meet the needs of others, if you Confirm a Scheduled Appointment and
then you request to be rescheduled, cancel, or no show for any reason,
within 24-hours of the Scheduled Appointment a $50.00 fee will be immediately due.
3. Out of an abundance of caution to our Staff and Patients:
a. If you are sick, i.e., cough, fever, chills, or sore throat,
you must be seen via Telemedicine;
b. Only the Patient being served will be allowed in the Clinic;
c. Only one Patient is permitted in the Clinic at a time
(you may be asked to wait outside if another Patient is still in the Clinic upon your arrival); and
d. All Persons visiting the Clinic MUST wear a Mask or Mouth/Nose covering in order to be seen.
Due to the scarcity of personal protective equipment,
we do not have extra face masks available.
4. Telemedicine Video Visits are available.
If you desire a Telemedicine visit instead of an In-Office Visit,
you must request the Video Visit 48-hours in advance.
You will be advised if you are eligible for a Video Visit appointment.
a. You must download, set-up, and test the
Application 24-hours in advance (it take 30 minutes),
b. Patients will need to log into the Application with a debit/credit card;
EVEN IF YOU MAY NOT BE REQUIRED to pay
a co-payment, deductible, and/or any outstanding fees;
c. Patients are to sign into the Application 15 minutes
before the Scheduled Appointment in case the
provider is available early;
d. Due to our limited schedule,
we will not be available to assist you with set-up of the Application
at the time of your Scheduled Appointment,
and a $50 no-show fee will be assessed;
e. Please be aware, the confirmation email for your
Telemedicine Video Visit may convert your appointment
time to Eastern Standard Time (EST).
All appointment times are scheduled using Arizona's Time Zone; and
