​Thank you for choosing our practice. We appreciate your business and look forward to serving you. To help us to serve you better, please note the following important information regarding your appointment.

Payment and Reservation Policy

A credit/debit card is required to reserve all appointments. Your card will not be charged until services are rendered.

Required Documentation

To secure your appointment, please send a screenshot of the front and back of your driver’s license, state ID, passport, or school ID; along with your insurance card within 4-hours. Use our secure text messaging app at 520-477-1815.

Completing New Patient Forms
 

​To complete our New Patient Registration Forms:

​

1. Online: Forms will be emailed or texted to you shortly (check your SPAM folder for the email from Healthy Wings Family & Psychiatric Healthcare<nobody@tebra.com>); or

​​

2. Website:  Please visit our "Patient Registration" page at www.HealthyWingsLLC.com to complete the forms for your scheduled visit. Carefully follow the instructions on the page to successfully submit your Patient Registration forms.

For Returning Patients

Returning patients do not need to complete additional forms unless you have a new insurance card or carrier. Please text us at 520-477-1815 the front and back of the new insurance card right away to confirm your in-network status.

Preparing for Your Appointment

  • Bring ​or have available all medications, vitamins, and supplements you are taking.

  • Bring ​or have available any relevant medical records or contact information for previous healthcare providers.

  • ​If In-Office, If you haven’t had lab work done in over six months, fast for 8-12 hours before your appointment. Drink plenty of water.

To get started with your Telehealth/Video Visit:

  1. Place your device in DO NOT DISTURB mode to avoid disconnection.

  2. Click here for visits with Dr. Jennifer Muhammad, DNP, NP-BC.

  3. Enter your first and last name.

  4. Allow permissions to use your device’s camera and microphone.

  5. Test your speaker.

  6. "Knock" to enter the waiting room.

 

Please sign-in at least 10 minutes early in case the healthcare provider is running ahead of schedule.

Cancellation Policy

We understand that circumstances may prevent you from keeping a scheduled appointment. If you need to cancel or reschedule, please call our office at least 24-hours in advance. A $75 fee applies for no-shows, cancellations, or to reschedule for any reason within 24-hours of the appointment.​

All patients coughing or feverish must wear a mask in the office.​

We are located at 2055 E. Southern Ave., Suite B, Tempe, AZ 85282. Our telephone number is (520) 477-1815.  Plenty of parking is available.  Our Office Suite is on the Right-hand side of the driveway entrance.​

​Payment Information

Payment, including co-pays and unmet deductibles, is expected at the time of service. For out-of-network insurance providers, full payment is expected at the time of service.

It is important to clarify that the fee(s) charged is for the time, expertise, and professional services provided, not for a guaranteed prescription(s) and/or specific outcome(s). Controlled substances and other medications are prescribed only when clinically appropriate and safe, based on professional medical judgment. Thus, all fees paid or to be billed on your behalf to your insurance(s) are non-refundable once services have commenced and/or have been provided by our professional staff members.

Insurance and Billing

We will provide an appropriately coded receipt for you to seek reimbursement from your insurance carrier. Or we may bill your insurance company as a courtesy, but you are ultimately responsible for payment, especially if payment is not received within 60 days. 

Please note that annual physical exams and well-woman exams are only covered if you are not experiencing any acute health issues or additional service fees may be due if experiencing acute health issues.

Accepted Payment Methods

We accept cash, checks, Visa, MasterCard, American Express, and Discover. We do not accept post-dated checks or payments.​
